Insurance Claims
Although Dr. Burks and Dr. Bryant are not "in-network" or "contracted providers" for any dental insurance company, we are happy to file all insurance claims and pre-determination of benefits for you. In order to do so, we must have current and accurate information from you or a copy of your current insurance card. If we do not have your card on file, or are unable to verify eligibility of benefits, you will be considered a self-pay patient. As a self-pay patient, our fee is expected to be paid in full at the time of service.
New patients are considered self-pay for their initial consultation. After this visit, we will be able to estimate future expenses. Please feel free to ask what your estimated out of pocket amount will be when calling for an appointment.
